JOB DESCRIPTION
NURSE

Position Title:  Nurse

Status:  Exempt

Minimum Qualifications: Must have at a minimum a license as a Practical Nurse meeting all licensing standards.

Maximum Qualifications:  Must have at a minimum a license as a Practical Nurse with a minimum of four years experience working with individuals having a disability and medical needs.

Supervised By:  Executive Director, Registered Nurse & Residential Service Manager

Compensation: Salary Schedule

Working Hours: Flexible hours as needed by clients receiving nursing services.  Could involve weekends, evenings, and holidays.

General Objectives: Provide quality nursing services, including medication distribution as per the client’s Individual Service Plan.

Duties and Responsibilities:

  1. Provide nursing oversight of assigned clients making necessary recommendations to coordinator, Residential Manager and/or Executive Director as prescribed in the Individual Service Plan.
  2. Provide oversight of administration of medications based on client’s needs and as outlined on prescription.
  3. Assist and advise coordinators in decision making regarding medical issues of client’s served.
  4. Participate in ISP meetings for those clients on medication or are in need of nursing services.
  5. Assure that all necessary documentation is completed and in client’s file; i.e., Nursing Care Plans, Medical Crisis Intervention Plans, Client Assessments, Medication Administration Reports, doctor/hospital forms, medication orders and other related forms per DDSD Standards.
  6. Be responsible and accountable for the in house safeguard and proper storage of medication as dictated by the Board of Pharmacy and other agency standards.
  7. Monitor vital signs as listed in ISP or as directed by physician.
  8. Advocate on meeting client’s medical needs.
  9. Inform caregiver of health care precautions and practices related to the individuals care; provide counseling and health teaching as necessary.
  10. Participate in the scheduling of routine and non-routine healthcare visits and supervise non-routine health visits as needed.
  11. Establish a positive rapport and working relationship with consumers, families, guardians, staff, co-workers, and other resource providers.
  12. Advocate for consumers at all times, while respecting the confidentiality of the consumer in all cases, as mandated by Federal Law.
  13. Promote a positive image of ELADC and those individuals it represents.
  14. Adhere to agency’s attendance and punctuality policy as well as maintain a positive attitude and keep clear lines of communication.
  15. Comply with and implement     ELADC policies and procedures, personnel policies and procedures as well as DOH standards and/or policies.

Physical, Mental, and Visual Skills:

  1. Good organizational skills.
  2. Ability to use logical thinking to solve problems.
  3. Ability to read written language, doctors reports and understand prescriptions and side effects.
  4. Ability to provide effective written and verbal instruction and provide comprehensive written reports.
  5. It is necessary to possess verbal and auditory skills.

Working Environments:

The major activities of the job are carried out in many types of conditions which vary.  Approximations:  Indoors 80%, outdoors 10%, and 10% in a car or van.
